Supportive living accommodation dedicated to providing evidence-based and individualized support to adults facing multifaceted barriers to living independently in community.

What is it?
Supported Housing is a long-term supportive living accommodation program for men and women with multiple barriers who experience difficulty living on their own in community.
We provide case management services that include ongoing assessment, advocacy and mental health monitoring. Resident Care Plans are developed according to residents' biopsychosocial needs and personal preferences.
Who is it for?
This program is for adults who experience difficulty living independently in the community. They must:

Agree not to use or store alcohol, cannabis or illegal substances within the building

Be able to live in a communal setting

Be able to manage finances independently, OR have a designated person to accept this responsibility

What is the housing setup?
The rooms are fully furnished, with shared, single-occupant bathroom facilities. Basic housekeeping services are provided. There are 72 Wellness Integrated Support Homes (WISH) beds in the facility.
The building is located in Castle Downs, with:

What is the program?
The Supported Housing Program strives to encourage clients to progressively become independent with their mental health, and includes components such as:

Recreation Therapy
- Spiritual supports

Medication administration

Nursing supports

Health Care Aide supports

Doctor and psychiatrist supports

Vocational pursuit supports

Safe community living
-
Connecting residents with community resources and building social networks

Food services - 3 meals (7 days a week) plus nutritious snacks

Other referral services as required
How do I apply?
Interested clients need a referral through the AHS CareHub, and must be matched to Supported Housing through Strata Pathways.
